Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment centered around healing and wellness. Nursing practices are based on evidence, current research, as well as hospital and department policies and procedures. Regulatory standards and professional guidelines also shape nursing practices. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaborative relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. This includes provision of nursing rounds, assessments, treatments, and facilitating admission, transfer, and discharge activities. Patient and family-centered education is provided, as well as care coordination and individualized treatment planning as applicable. The RN performs and documents a comprehensive assessment in accordance with the RN's scope of practice. The assessment considers the patient’s culture, age, and developmental level. Identifying patient risk factors and presenting complaints is a key responsibility. The RN collects both objective and subjective data and verifies initial admission assessments. A problem list is developed, prioritizing patient needs based on input from the inter-professional team as appropriate. The RN designs actions and interventions relevant to the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care. Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy. The RN explains procedures, treatments, and processes to patients and families. Initiating actions to alleviate patient stress and discomfort is essential. Recognizing and communicating changes in patient condition appropriately is a key task. Ongoing assessments and documentation of responses to procedures and therapies are conducted using the nursing process. The RN delegates tasks appropriately and oversees team members in the performance of activities regarding patient care. Monitoring accuracy and completeness of delegated patient care is an important responsibility. Identifying and addressing educational needs of patients related to prioritized problems according to their level of comprehension is required. The RN facilitates the patient discharge process by executing discharge orders in a timely manner. Input into patient/nurse assignments is provided while considering staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is expected. Promoting a positive group dynamic and healing environment through words and actions is a goal. Nursing excellence is assured in accordance with accepted standards, evidence-based practice, current research, and a caring philosophy. Active participation in department and unit quality improvement activities and core measures is necessary. The RN uses a holistic perspective regarding patient care.
