Work Environment

This role is situated primarily in patients' homes throughout the Perth Amboy area, where you will implement tailored therapeutic programs in a comfortable and familiar environment. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, developing and implementing individualized treatment plans to address various functional deficits.
• Utilize a variety of treatment modalities, including manual therapy, therapeutic exercises, and patient education, to promote recovery and enhance functionality.
• Foster excellent relationships with patients and their caregivers by effectively communicating treatment goals and progress.
• Work collaboratively with a multidisciplinary team to share insights and strategies that maximize patient recovery efforts.
• Monitor patient progress, adjusting treatment plans as necessary to ensure optimal outcomes aligned with physician recommendations.
• Maintain accurate and timely documentation for all patient interactions and treatment sessions, adhering to compliance and regulatory standards.

Patient Population / Client Focus

Our practice caters to a diverse patient demographic, including individuals recovering from orthopedic surgeries, neurological conditions, pediatric issues, and sports-related injuries. Your role will involve customizing your approach to address the unique needs of each patient, thereby ensuring a respectful and effective care experience tailored to their specific rehabilitation goals.

Qualifications

• Bachelor's degree in Physical Therapy (DPT).
• A valid physical therapy license in the State of New Jersey.
• A graduate from an accredited program recognized by the American Physical Therapy Association (APTA) or CAPTE.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong written and verbal communication skills.
• Proficient in computer skills, including word processing, to facilitate documentation and communication.

Preferred Experience

We are looking for candidates with at least two years of experience in a physical therapy setting. Experience in home health care or outpatient rehabilitation is a significant advantage. The ideal candidate will demonstrate a strong commitment to patient-centered care and possess a passion for building relationships that promote healing and recovery.

Compensation and Benefits
